Payment for initial examination and maintenance appointments is due at the time of service.

Our office participates with most PPO plans, please ask about yours. Patients using insurance are expected to pay co-payments and/or deductibles at the time of service.
Mamsi insurance will be filed at the time of visit. Please note that our office is not a participating provider with the above plan.
